Ages 16-17: Must bring signed ORIGINAL Blood Bank parental consent form.
All Donors: Bring NRIC, Driving Licence, OR Blood Donor Card.

By registering for and participating in this event, you hereby consent to interview(s), photography, audio recording, video recording and its/their release, publication, exhibition, or reproduction to be used for news, web casts, promotional purposes, telecasts, advertising, inclusion on web sites, or for any other such purpose(s) that the Sengkang North C2E or the People's Association (“PA”), its vendors, partners, affiliates and/or representatives deem fit for its use.
