DECLARATION
1)	I am aware that my attendance in the course/activity involves a certain amount of risk.  

2)	I understand that I will have to cooperate fully with the staff and diligently comply with the staff's instructions and all safety systems. 
 
3)	I declare and confirm that I have read and fully understand all the parts in this course/activity registration form and I hereby accept the risks involved in the course/activity as disclosed in the information provided by PAssion WaVe. 
 
4)	I will therefore not hold PAssion WaVe or People's Association or their servants and agents responsible for any damage to or loss of property or any injury or loss of life which may be sustained by me during the course/activity or arising from any cause in connection with the course/activity where such damage to or loss of property or any injury or loss of life is not caused by the negligence or wilful act or omission of PAssion WaVe or People's Association or their servants and agents.

6)	I have completed the GAQ (GAQ can be found in the attached file or under item (2) here: https://www.pa.gov.sg/Our_Programmes/PAssion-WaVe) and/or have sought the doctor's advice on where I am unsure and have determined that I am fit to take part in the course/activity.
