I agree not to hold Bukit Batok Zone 8 RN, Bukit Batok Community Club, the People's Association, or their staff and volunteers liable for any damage to or loss of property, or any injury, or loss of life sustained during or arising from the course/activity, unless such damaged, loss, or injury is due to their negligence or willful misconduct. 5. I confirmed that I am fit to participate in this course/activity. 6. I declare that all information I have provided is true and accurate. 7. I am under 21 years of age, I have obtained consent from my parent/guardian to participate and to agree to the terms above.
